Use this form to provide as much information as possible about the workplace safety or health hazard, and click on the submit button at the end of the form. Your information will be submitted via e-mail to our Safety Violations Hotline Administrator.

If you wish to remain anonymous, you may do so:

  • If you provide your contact information, we will keep your name confidential.

  • If you provide your e-mail address, without your name, we will be able to contact you and keep your identity confidential, since e-mail addresses have been made confidential by the Texas Legislature.

  • If you do not choose to provide your contact information, we will be unable to update you on the status of the hazard correction.

Under most circumstances, individual e-mail addresses are protected by the Texas Public Information Act. Sharing this information for purposes of processing your information does not waive these confidentiality protections. Access and Correction of Personal Information

With few exceptions, you are entitled to be informed about the information that the Texas Department of Insurance (TDI) collects about you. Under sections 552.021 and 552.023 of the Texas Government Code, you have a right to review or receive copies of information about yourself, including private information. However, TDI may withhold information for reasons other than to protect your right to privacy. Under section 559.004 of the Texas Government Code, you are entitled to request that TDI correct information that TDI has about you that is incorrect.
